The use of enterosorption with Enterosgel medication in combination therapy of children with acute pyelonephritis reduces the toxic load on the neutrophilic granulocytes and contributes to the preservation of functional activity of the given cells at subcompensated level, reducing the risk of generalization of infection and the development of associated complications.
ln order to assess the characteristics of the medical device ENTEROSGEL, establish the method of its use and the safety for users, this report was drawn up in the form of a critical evaluation of the relevant published clinical studies conducted with ENTEROSGEL and with other at least partially comparable medical devices already placed on the European market.

Nowadays the problem of interaction between the environment chemical composition and the health of the population is a challenge not only in Russia but also in other parts of the world. The level of overall health and quality of the environment (taking into account natural and anthropogenic factors) are to different extent determined by biogeochemical features in different regions of Russia. With intensification of industrial and human activities, urban growth, creation of "megalopolises" and major industrial centers the load has increased so intensively that a special ecological environment was formed, with high concentrations of anthropogenic products – i.e. with a serious pollution of air, soil and water basins.
The term “enterosorption” has been proposed to define a method of sorption therapy based on daily oral administration of “enterosorbents”. Enterosorbents were known in the past because the tradition of eating natural clays in case of poisoning, for improvement of well-being, jugulation of diarrhea, and, possibly, for treatment of deficiency in microelements, goes back to bygone times, and as a phenomenon known as geophagia it was observed particularly in primates [6, 7]. However, Enterosgel, comparing to other sorbents has unique hydrophobic and selective sorption properties, which made it effective and safe in wide range of medical applications. Enterosgel has proven it’s safety and efficacy during 35 years of clinical studies and use in major hospitals of the Soviet Union, it is recommended in the official guidance of Ministry of Health for treatment of various diseases. Since 2008 Enterosgel is certified as a medical device class IIa and available as an OTC treatment in Europe.

Acute enteric infections (AEI) are among the widely prevalent infectious diseases worldwide. The highest incidence of intestinal infections occur in pediatric populations . The main reason is biodiversity pathogens AEI: bacteria, viruses and protozoa from different taxonomic groups. Bacterial AEI can cause the genera Salmonella, Shigella, pathogenic strains of E. coli, spp. Clostridium, spp. Campylobacter spp., spp. Staphylococcus spp. Klebsiella, etc. Viral AEI pathogens are group A rotaviruses and noroviruses, astroviruses, F adenoviruses, sapoviruses. Discusses the role of group C rotaviruses, bocaviruses, Aichi virus, parechovirus, coronaviruses, etc. in the development of acute gastroenteritis.

Gastroenteritis severity is linked to etiology, and rotavirus is themost severe infectious agent and is frequently associated with dehydration. Dehydration reflects severity and should be monitored by established score systems. Investigations are generally not needed. Oral rehydration with hypoosmolar solution is the major treatment and should start as soon as possible. Breast-feeding should not be interrupted. Regular feeding should continue with no dietary changes including milk.

According to recent epidemiological and toxicological data, children living in poor sanitary conditions develop abnormal physical characteristics, such as morphological abnormalities and chronic diseases of the Central Nervous System and the respiratory, cardiovascular, musculoskeletal, endocrine and digestive Systems. Among the substances of industrial origin, an important role is played by metals that accumulate in the tissues and organs. The toxic effects are due to the type of metal, its concentration, the concurrence of other pathological factors and the general health conditions (immune reactivity, hypersensitivity, etc.). The study was conducted on 236 children suffering from mild asthma (experimental Group) living in areas close to metallurgical plants.

Convincing evidence has accumulated showing that ondansetron reduces the risk for vomiting; however, a clearance on safety in children is needed. New evidence has reconfirmed that in Europe, where zinc deficiency is rare, there is no benefit from the use of zinc. New data, although mainly from outside of Europe, have reconfirmed that either smectite or racecadotril is an effective adjunctive therapy to oral rehydration. There is a clear effect of using certain probiotics, such as Lactobacillus GG or S. boulardii.

Treatment method of intestinal adsorption with Enterosgel made in the early days in newborn infants with isoimmune hemolytic disease is pathogenetically substantiated, provides more favorable course of the pathological process, the positive dynamics laboratory markers leads to a lower number of exchange blood transfusions and phototherapy sessions, reduces the risk of infection in newborns and shortens the period of inpatient stay. The intake of Enterosgel is safe, its implementation is simple and affordable.

The main properties of enterosorbent Enterosgel®(paste) were analyzed. It was found that sorptiondetoxication action of this medication improves the state of gastrointestinal tract and mucous membranes, and evokes a number of positive "distant" effects, such as inhibition of lipid peroxidation, decrease in the content of circulating immune complexes and pro-inflammatory cytokines, subcompensation of immunodeficiency, activation of liver and kidney function, etc. Besides, this medication is quite safe. The inclusion of detoxicant Enterosgel® in the combined therapy allows to significantly improve the results of treatment of various diseases in children.

Prevalence of urinary system diseases in Ukraine is 37 per 1000 children and tends towards constant growth. Among the urinary tract infections pyelonephritis (PN) is the most serious problem, which occupies a leading place among nephrology pathologies of childhood [3, 4]. In the recent years, an increase of recurrent and latent disease types is often accompanied by development of intoxication syndrome, probably caused by agents of toxin production, as well as by response of the body, with the development of endotoxemia, metabolic and immune distress.
Acute intestinal infections (AII) is one of the important health issues in both developing and developed countries, being the next widespread among mass infectious diseases after respiratory infections. Despite the progress made in improving diagnostics and treatment tactics, acute intestinal infections are widespread - they are transferred annually by at least 500,000 children (Onishchenko G.G., 2006; Uchaikin V.F., 2007; Feklisova L.V., 2007). In recent years there has been a clear trend in etiological significance of pathogens causing acute intestinal infections in
children (Gorelov A.V., Milutin L.N., 2006). There is no doubt that among them a leading role in the world belongs to viruses causing 50-80% of AII in children (Glass R., 2005; Parashar V., 2003 Podkolzin A.T., 2005). AII represent the greatest danger for the children in the first years of life due to high rates of morbidity and mortality worldwide (Sharapova O.V., Korsun A.A., 2003; Glass R.I. et al, 2000; Wilhelmi I. 2003.).

The problem of elimination of toxic substances from the body is one of the most pressing in pediatrics. Hemosorption, hemodialysis and plasmapheresis has been successfully used in pediatric practice for a long time; however, the risk of their use may exceed expediency in some cases (for example, in case of moderate toxicity). Pediatricians have been always interested in non-invasive detoxification methods, in this context we have piloted Enterosgel, a new enterosorbent, in addition to infusion therapy, plasmapheresis, and in some cases as the unique detoxification therapy.

The performed analysis showed that administration of Enterosgel in treatment of the patients with enteroidea allowed reducing the duration of hospital stay by 2-3 days, which is due to a faster normalization of stool and disappearance of the guiding symptoms. No adverse and hypersensitivity reactions were detected upon its administration.

The medication Enterosgel offered by the enterprise "SILMA" approved itself as the efficient agent for treatment of various allergic pathology. Taking of Enterosgel by patients having the atopic genesis of bronchial asthma and dermatitis helps to arrest the broncho-obstructive syndrome and skin syndrome due to adsorption of allergens. Enterosgel allowed patients to withdraw from antihistamine drugs, reduce the administration time of bronchial spasmolytics. It showed a high clinical activity in the therapy of both acute and chronic digestive diseases.
Thus, Enterosgel is the efficient supplement to conventional treatment of allergic and gastrointestinal diseases in children.

The results of clinical and laboratory research confirmed high efficiency of use of Enterosgel in the treatment of newborn infants with rotavirus infection. Regarding the data collected, it is possible to draw a conclusion about the possibility and the need to use Enterosgel for treatment of diarrhea caused by rotavirus in newborns and prevention of infectious complications.

Every year the range of problems associated with use of sorption technologies in various fields of medicine is expanding. Growing use of sorption processes, in turn, leads to improvement of the properties of older types of sorbents and creation of new sorbents with improved properties and enhanced sorption selectivity toward certain substances. Comparative medico-biological studies of various polymeric materials has shown prospectivity of medical use of organopolysiloxane adsorbents, particularly of polymethylsiloxane (PMS) and its hydrogel, Enterosgel, which consists of a spatially cross-linked porous matrix with a rigid globular structure formed by siloxane chain containing methyl and partially uncondensed hydroxyl groups in silicon atoms. The presence of hydrocarbon radicals provides high affinity of Enterosgel to organic substances (organophilic properties), that is to the toxic metabolites of various nature. In contrast to the known carbon sorbents and highly dispersity silicas, Enterosgel is characterized by apparent selectivity.
